Description of the field values for the problem task model form.
| Field | Description |

| Name | Unique name for the problem task model. |
| Problem task type | Option to map the problem task model to a problem task type. The default selection is Model when you create a custom problem task model. |
| Default problem task model | Option for enabling the problem task model as a default model on the problem request form. |
| Active | Option for specifying that this model is available for selection when creating a problem request. |
| Description | Detailed description of the problem task model. |
| Advanced Security | Option to enable role-based access controls and user criteria to tailor the problem creation landing page views. When you enable Advanced Security, the Available For, Not Available For, and Can write tabs appear next to the Model States tab. |
| Read Roles | Option to define the roles to view the problem task model. |
| Write Roles | Option to define the roles to edit the problem task model. |
