Deviation form

The following table describes the field values for the Deviation form.

FieldDescription
Short descriptionBrief description of the deviation you’re creating.
DescriptionDetailed description of the deviation you’re creating.
Functional locationIndustrial facility work area where the deviation is observed. This field is defined using the ISA-95 standard. Automatically filled based on the logged-in user's worker profile.
EquipmentMachine to which the deviation relates.
Have you retained one or more parts?Only available if the Classification is set to Breakdown.
ClassificationType of the deviation. Options are:- Deviation - Breakdown - Process failure - Defect When initiating a breakdown, this field is automatically set to Breakdown.
CategoryGroup to which the deviation belongs based on characteristics. Options can be:- Mechanical - Electrical - Pneumatic - Hydraulic - Environmental - Defect - Other
SubcategorySubcategory of the deviation.
Active materialActive material to which the deviation relates.
FailureAvailable options vary depending on the selected functional location or equipment.
OriginTask from which the deviation originates.
ImpactImpact of the deviation. Options are:- 1 - Safety - 2 - Quality - 3 - Reliability - 4 - Operations - 5 - Other
UrgencyUrgency of the deviations. Options are:- 1 - Critical - 2 - Important - 3 - Routine - 4 - Deferred
PriorityPriority that is set automatically based on impact and urgency. Options are:- 1 - Direct - 2 - This shift - 3 - Today - 4 - Within 7 days - 6 - Unplanned For more details, see Priority matrix for deviations.
Assignment groupTeam or department responsible for completing the task.
Assigned toUser to which the deviation has been assigned. If not set, the deviation is automatically assigned to the user that opened the deviation.
Additional assignee listList of users to be notified or contributing to the task, but not as the primary assignee. Available options vary depending on the selected functional location or assignment group.
Resolution codeResolution code for the impact. Options are:- None - Not fixed - Fixed by operator - Fixed by maintenance
ResolutionDescription of how the deviation has been resolved.
AttachmentsAttachment that can be used for descriptive purposes.
