Use the Assessment Metric Type New Record form to create an attestation template to create the attestations. Use the template form to create a record.

| Name | Name of the template. |
| Description | Information about the template. |
| Active | Option to enable the template. |
| State | State of the assessment. The available options are as follows:- Draft - Published This field is auto-filled. |
| Metric categories | Category that represents a theme for evaluating assessable records in a given metric type. It displays the name of the template, description of the category, its weight, order, and so on.
