Report an AI case form

Use the Report an AI case form in the Anonymous Report Center to report an AI case with the necessary details. After you submit the AI case report, mandatory fields and input format are validated before submission.

See the following table for a description of the field values.

Note: When submitting an anonymous report, don’t include personal data about yourself or other individuals that could directly or indirectly reveal identity (for example, names, email addresses, or phone numbers).

Basic information
Type of AI issue	Category of the AI issue being reported.The following options are available. Fairness & Transparency (Lack of explainability in AI decision-making, leading to unfair outcomes.) Misinformation & Deepfake (AI-generated false content violating copyrights, patents, or trademarks.) IP Theft & Copyright Infringement (AI-generated content violating copyrights, patents, or trademarks.) Unauthorised AI Model Usage (Use of AI models outside agreed licensing terms.) AI System Malfunction (AI failures leading to unintended consequences in critical applications.) Data Poisoning (Manipulation of training data to mislead AI model behavior.) Bias & Discrimination (e.g., racial, gender, socioeconomic biases.) Model Inversion & Extraction (Unauthorised access or reverse engineering of AI models.) AI-induced Physical or Psychological Harm (Physical harm example -Self-driving car accidents. Psychological impact example - misinformation.) Adversarial Attacks (Deliberate manipulation of AI models to produce incorrect results.) Privacy & Data Protection Cases (Unauthorised collection, processing, or misuse of personal data.) Autonomy & Human Oversight (AI acting independently without appropriate human intervention, causing ethical concerns.) Note: This field is mandatory.
Brief summary of the AI case	Short, descriptive summary of the concern. For example, `Incorrect AI recommendation shown to customers`. Note: This field is mandatory.
Date you became aware of this event	Date when you first became aware of the AI-related issue. By default, the current system date populates this field. Input the date in the following format: YYYY-MM-DD. The date provided cannot be a future date.
Describe what happened in a few words	Provide a detailed description of the AI case. Explain what happened, why it may involve improper or risky use of AI, and any potential impact on the organization, customers, or operations. Anonymous reports accept standard text characters only. Unsupported characters, including spaces, symbols, scripts, and code, are automatically removed during submission. Only alphanumeric characters and `@ . _ -` are retained. Note: This field is mandatory.
Rate this case as criticality	Indicate the severity or criticality level of the AI case.The following options are available. - Critical - High - Moderate - Low - Planning
Nature of impact
Business units involved or impacted	Business units affected by the AI case. For example, HR, Operations, Customer Service, or Risk & Compliance.
Departments involved or impacted	Departments affected by the AI case, such as Marketing, Legal, or Operations. Enter `Skip if unknown` if applicable.
Location/jurisdiction	Location where the event occurred, such as an office site, department area, internal system, application, or customer-facing environment.
Text verification	Enter the CAPTCHA verification code on the screen.
