Ethics complaints intake form in the Anonymous Report Center portal
Employees can use the Ethics Complaints intake form in the Anonymous Report Center (ARC) portal to report an ethics violation incident. Different sets of fields appear on the form based on the active record producers.
The following fields appear on the Ethics Complaints form in the ARC portal when the record producer mapped to the Ethics [sn_lg_ops_ethics] table is active.
| Field | Description |
|---|---|
| Complaint type | Type of complaint or allegation that violated the ethics policy of the organization. |
| Detailed description | Detailed information about the incident, such as where and when it happened. |
| Involved parties | Users who are the subject of the complaint.Enter the names or email IDs of the involved parties, each name separated by a comma. |
| Approximate date of event | Tentative date when the incident happened. |
| I understand that the information I'm providing, including any sensitive or personal details, will be reviewed by the appropriate teams | Option to acknowledge submitting sensitive information or personal details of people involved through the request. |
The following fields appear on the Ethics Complaints form in the ARC portal when the record producer mapped to the Universal Request [universal_request] table is active.
