| What title would you like to use to describe your invention | Title of the invention. |
| Briefly describe what problem your invention solves | Description of the issue that the invention will help to resolve. |
| Describe the invention | Detailed description of the invention. |
| Approximate date of conception of invention | Date when the invention was conceived. The date must be either the current date or earlier. |
| Invention publicly disclosed? | Indicates whether the invention has been disclosed publicly. If the choice is Yes, additional fields request detailed information about the disclosure.- Invention disclosure method - How the invention was publicly disclosed. - First public disclosure date - Date when the first public disclosure of the invention was made. |
| Invention was implemented into a customer product or is planned on being implemented into a customer product? | Indicates whether the invention is already used anywhere. If the choice is Yes, additional fields request detailed information about the implementation.- Past or future date of release of the product - Past or future date when the product releases. - Specify the name of the product - Name of the product where the invention is used. |
