Work on a safety incident

Work on a safety incident assigned to you to collect incident information, ensure that any outstanding tasks are completed, and resolve the incident.

Before you begin

Role required: sn_ohs_im.manager, sn_ohs_im.agent, or sn_ohs_im.operations_manager

About this task

Use the Report an incident playbook available with Health and Safety Incident Management to manage safety incidents and resolve them. You can access the Report an incident playbook tab when a safety incident is assigned to you.

Note: To work in a tab-based experience for safety incidents, your administrator can disable the playbook through Health and Safety properties. For more information, see Disable the safety incident playbook.

Procedure

  1. Navigate to All > Health and Safety > Health and Safety Workspace.

  2. Select the incident management icon (

  1. In the Lists tab, select Safety Incidents and then All.

  2. In the list, select the incident to work on.

  3. Select the Report an incident tab.

    This playbook guides you through the steps that are involved in resolving an incident.

  4. Record the information related to the incident using the playbook activities and tasks.

    Selecting Mark complete marks an activity as complete and makes the next activity or stage available in the playbook. However, you can also select Skip to move to the next activity. You can go back to previous activities and update them later.

    • Enter incident summary, to add a summary of the incident such as the time and date the incident occurred on, severity, and the category of the incident. For more information, see Health and safety incident form.
    • Add event details, to add event location and sequence of events. For more information, see Log sequence of events.
    • Add people involved, to add people involved in the incident such as an injured person, witnesses, and collaborators. For more information, see Add or modify people involved.
    • Add injury and illness, to log an injury and illness report for each affected person during the incident. For more information, see Log an injury and illness
    • Add assets involved, if an asset was involved in this incident. For more information, see Associate an asset to the incident.
    • Add root cause analysis, to perform root cause analysis for the incident. For more information, see Perform root cause analysis.
    • Add actions based on your root cause analysis.

  5. Record your findings that you gathered about the incident.

    You can use these sections to add your findings:

    • Attachments section to attach findings in the form of image, video, and documentation such as a signed witness statement.
    • Activity section to add findings in the form of free text. You can also use this section to add links to related knowledge base articles.
    • Research content related to the incident by selecting the Agent assist icon (

Agent assist searches the knowledge base using the text from the incident’s **Short description** field and shows relevant articles.
  1. Generate a report for the safety incident, if needed.

    For more information, see Generate a report for a safety incident, observation, or injury.

  2. Close the safety incident after all activities are complete.

    The incident moves to the Closed Complete state.

  3. Add hazards and log sequence of events for a safety incident
    Add hazards andlog a sequence of events for a safety incident as part of its investigation.

  4. Manage people involved in a safety incident
    Add a person involved or modify the information of an existing person involved in a safety incident.
  5. Log an injury and illness
    Log an injury and illness that resulted from a health and safety incident.
  6. Associate an asset with a safety incident
    Associate an enterprise asset with a safety incident.
  7. Perform root cause analysis for a safety incident
    Perform a root cause analysis (RCA) for a safety incident as you go through its investigation.
