Application service form

An application service is a configuration item (CI) that represents the operational view of a deployed application or your technology landscape. It logically models all technical components such as servers, databases, middleware, and configurations, that work together to deliver a specific business capability. Application services are linked to business applications in the CMDB.

Support groupName of the user group who supports the application service.
Supported byName of the user who supports the application service.
Managed byName of the user who manages the application service.
Managed by groupName of the user group who manages the application service.
EnvironmentName of the environment such as Development, Production or Test.
CompanyName of the company.
ManufacturerName of the manufacturer.
Model IDModel ID associated with the service.
Model numberNumber of the Model.
CategoryThis field is auto-populaged as Business Service.
Sub-categoryThis field is auto-populaged as Service.
DepartmentName of the department.

Discovery sourceDiscovery source of the application service.
First discoveredSelect the first discovered date.
Most recent discoverySelect the most recent discovery date.
Correlation IDEnter the co-relation ID.
Skip syncOption to select to skip the sync. This is a read-only field.

Life Cycle StageSelect the life cycle stage of the application service.
Life Cycle Stage StatusSelect the status of the lifecycle stage.
Attested byName of the user who attested.
Attested DateDate when the attestation is done.
Attestation StatusSelect the attestation status as Attested or Rejected.
Install StatusReason for the planned disposition decision.
Operational statusSelect the operational state of the application service.
Fault countNumber of the fault count.
Maintenance scheduleName of the maintenance schedule.
ScheduleSelect the schedule for maintenance.
Requires verificationOption to select to request verification. This is a read-only field.
